Query: My friend's wife, 3 times be pregnant & have abortions. She had a blood test that showed that she is CMV infected and it is about 196. She took acyclovir. Now it becomes about 67. Could she be pregnant without abortion again? Or may this ratio increase again & abortion happened again?

Answer: Is it a CMV IgM or IgG? If it is CMV IgM, it suggests active infection and chances of spread to the fetus are high. If it is IgG, it suggests past infection. However, one cannot say that the infection has resolved by a falling titre. To be absolutely sure that the infection is under control, a CMV PCR test in the urine or blood would be useful. If CMV PCR or viral isolation is negative, the chance of infecting the fetus is negligible. One needs to evaluate the cause of multiple abortions as CMV infection is unlikely to have affected 3 offspring. Conditions such as antiphospholipid antibody, chromosomal antibodies, and other infections should also be ruled out
